If you are a grandparent or another family member inquiring about a possible baptism please call the parish office 847-468-6900 with any questions you may have. Do not fill out this form. The scheduling of a baptism is a process that only begins after one of the child's parents fills out this Request Form and submits it to the parish office.

Jesus commanded his Church to "Go, therefore, and make disciples of all nations, baptizing them in the name of the Father, and of the Son, and of the holy Spirit, teaching them to observe all that I have commanded you." (Math 28: 19-20).

We are overjoyed that you would like to baptize your child promising both God and your child your best efforts to raising him/her in the Catholic Faith. We also appreciate your trust in St. Laurence Church to help you fulfill that promise.

¡Felicidades! Le damos la mas cordial bienvenida a usted y a su familia a la Iglesia de San Lorenzo para la celebración del Sacramento del Bautismo.

Jesús mandó a su Iglesia que "Vayan, pues, y hagan discípulos de todas las naciones, bautizándolos en el nombre del Padre, y del Hijo, y del Espíritu Santo, enseñándoles a observar todo lo que os he mandado". (Mt 28:19-20).

La programación de un bautismo es un proceso exclusivamente solicitado por los padres del niño o uno de los padres al llenar la solicitud y enviarla a la oficina parroquial.

Es un gusto saber su deseo en bautizar a su hijo, comprometiéndose tanto con Dios como con el niño haciendo sus mejores esfuerzos para criarlo en la fé católica. También agradecemos su confianza en la Iglesia de San Lorenzo para ayudarlo a cumplir esa promesa.

Una vez que tengamos sus datos personales e información de acuerdo a sus circunstancias, podremos ayudarlo rápida y eficientemente a prepararse para un bautismo de su hijo significativo y fructífero. Por favor complete el formulario de solicitud de Bautismo a continuación.
